About Musculoskeletal Injuries
-
Lumbar Spinal Stenosis (LSS) is a clinical condition characterized by the narrowing of the spinal canal in the lower back. This narrowing is often a result of aging and is commonly associated with osteoarthritis, which can lead to the pinching of the spinal cord or nerve roots. When the narrowing is minimal, it may not cause any symptoms. However, significant narrowing can compress the nerves, leading to a range of issues. The clinical symptoms of spinal stenosis typically worsen over time as the nerves become increasingly compressed.
-
Early stages of lumbar spinal stenosis may be asymptomatic. However, in most individuals, symptoms gradually develop over time. Common symptoms include:
Back Pain
Burning Pain: Radiating into the buttocks and down the legs (sciatica)
Numbness, Tingling, Cramping, or Weakness: Primarily in the legs
Loss of Sensation: Particularly in the feet
Foot Drop: A weakness in the foot that causes it to slap down when walking
Loss of Sexual Function
In more severe cases, pressure on the nerves in the lumbar region can lead to a serious condition known as Cauda Equina Syndrome. If you experience any of the following symptoms, it is critical to seek medical attention immediately:
Loss of Bowel or Bladder Control
Severe or Increasing Numbness: Especially between the legs, inner thighs, and the back of the legs
Severe Pain and Weakness: Spreading into one or both legs, making it difficult to walk or rise from a chair
To diagnose lumbar spinal stenosis, your healthcare provider will begin by asking about your symptoms and conducting a thorough physical examination. During the exam, your provider will look for signs of spinal stenosis, such as loss of sensation, muscle weakness, and abnormal reflexes.
-
The following tests may be used to confirm the diagnosis:
X-rays of the Lumbar Spine: X-rays can reveal bone growths (spurs) that may be pressing on spinal nerves or show narrowing of the spinal canal.
Imaging Tests: A CT scan or MRI scan provides a more detailed view of the spinal canal and nerve structures, helping to identify any compression or abnormalities.
Additional Studies: Your healthcare provider may also order other tests, such as a bone scan, a myelogram (a CT scan performed after injecting contrast dye), or an EMG (electromyography), which tests the electrical activity of muscles.
The Treatment Journey
-
if you have Lumbar Spinal Stenosis, a variety of healthcare professionals can assist in your care, including arthritis specialists, neurologists, surgeons, and physical therapists.
Treatment Options:
Physical Therapy:
Physical therapy may involve exercises to strengthen the muscles of the back, abdomen, and legs. It can also include instruction on how to perform activities safely, the use of braces to support the back, stretching exercises, and massage therapy.
Medications:
Medications may include nonsteroidal anti-inflammatory drugs (NSAIDs) to relieve pain and reduce swelling. Steroid injections can also be administered to decrease inflammation and provide relief.Surgical Treatments:
Surgery is typically considered a last resort, except in emergencies such as Cauda Equina Syndrome. Surgical options may involve removing bone spurs, widening the space between vertebrae, or stabilizing the lower back by fusing together some of the vertebrae.
Alternative Therapies:
Some individuals may find relief through alternative treatments such as acupuncture and chiropractic care.

Stem Cell Therapy (SCT) is not a new treatment approach, but its popularity has surged in recent years due to its exceptional benefits.
Mesenchymal Stem Cells (MSCs):
MSCs are stem cells derived from donor blood or other tissues. After extraction, these cells are expanded to enhance their efficacy. Our primary source of stem cells is Wharton’s Jelly Mesenchymal Stem Cells (WJ-MSCs), a class of stem cells known for their high differentiative potential, immune-privileged status, and ease of collection. The therapeutic utility of WJ-MSCs is closely tied to their regenerative and immunomodulatory properties. These cells are capable of immune suppression and immune evasion, making them optimal candidates for cellular therapies in allogeneic transplantation.
-
After successful processing, these cells are injected into the targeted area. In the case of patients suffering from spinal stenosis, physicians use medical imaging to accurately locate the site of treatment. Once administered, the stem cells help reduce pressure and loosen the tight ligaments around the nerves, alleviating pain and irritation.

Exosomes and stem cell therapy are emerging as potential treatments for lumbar spinal stenosis, a condition characterized by the narrowing of the spinal canal in the lower back, leading to pressure on the spinal nerves and causing pain, weakness, or numbness. Both exosomes and stem cells focus on regenerative healing and reducing inflammation. Here’s how they are thought to work:
1. Exosomes in Lumbar Spinal Stenosis Treatment
Exosomes are small extracellular vesicles released by cells, particularly stem cells. They play a critical role in cell communication and the regulation of various biological processes. In regenerative medicine, exosomes are believed to have the following effects:
- Anti-inflammatory Effects: Exosomes contain proteins, RNA, and microRNA that modulate the immune response, reducing inflammation in the damaged or compressed nerves and tissues caused by spinal stenosis.
- Tissue Repair and Regeneration: Exosomes promote healing by encouraging cell proliferation and differentiation. In lumbar spinal stenosis, they can potentially help repair damaged discs, ligaments, and surrounding structures, aiding in the regeneration of tissues compressed by the narrowing spinal canal.
- Angiogenesis (Formation of New Blood Vessels): Exosomes promote the growth of new blood vessels, which can improve blood supply to injured or degenerated areas of the spine, enhancing the body's ability to heal.
- Reduction of Scar Tissue: Exosomes may help minimize fibrosis (formation of scar tissue) around the spinal nerves, which can lead to improved nerve function and reduced pain.
2. Stem Cell Therapy in Lumbar Spinal Stenosis Treatment
Stem cell therapy involves using mesenchymal stem cells (MSCs), which have the potential to differentiate into various types of tissues, including bone, cartilage, and muscle. These stem cells are often sourced from the patient’s bone marrow, adipose tissue, or umbilical cord tissue. Here’s how they may help in treating lumbar spinal stenosis:
- Regeneration of Degenerated Discs: Stem cells can potentially differentiate into disc cells, promoting the regeneration of intervertebral discs that have degenerated, which is a common contributing factor to spinal stenosis.
- Reduction of Inflammation: Stem cells have strong anti-inflammatory properties, which can help reduce the inflammation that contributes to nerve compression in spinal stenosis.
- Promotion of Tissue Healing: Stem cells secrete various growth factors and cytokines that promote healing and repair of damaged spinal structures, such as ligaments, cartilage, and bone, leading to improved stability and reduced nerve compression.
- Restoration of Nerve Function: By reducing inflammation, promoting tissue repair, and stimulating angiogenesis, stem cells may help improve the function of nerves that have been compressed due to stenosis. This could result in pain relief, improved mobility, and better quality of life.
Combination of Exosome and Stem Cell Therapy
In some cases, exosome therapy may be combined with stem cell therapy to enhance the regenerative effects. Exosomes can boost the activity of stem cells and provide additional anti-inflammatory and regenerative signals. Together, they may create a more potent healing environment, addressing both the underlying causes (disc degeneration, inflammation, and tissue damage) and the symptoms (nerve compression and pain) associated with lumbar spinal stenosis.
Advantages of These Therapies
- Minimally Invasive: Both exosome and stem cell therapies can be delivered via injections, avoiding the need for more invasive spinal surgery.
- Regenerative Potential: Unlike traditional treatments like steroid injections or surgery, these therapies aim to heal and regenerate damaged tissues rather than just manage symptoms.
- Reduced Need for Surgery: For some patients, these therapies may help avoid or delay spinal surgeries, which carry higher risks of complications and longer recovery times.
Current Limitations and Research
While these therapies are promising, more research and clinical trials are needed to fully understand their effectiveness and safety for lumbar spinal stenosis. These treatments are still considered experimental, and their availability may be limited depending on location and regulatory approvals.
In summary, exosome and stem cell therapies offer regenerative healing, tissue repair, and anti-inflammatory effects, which can help address the underlying causes of lumbar spinal stenosis. However, they are still in the early stages of clinical use and require further investigation to confirm their long-term effectiveness.

Significance of Hyperbaric Chambers post-treatment:
Hyperbaric Chambers are oxygen-rich chambers that can increase the concentration of Stem Cells in the body.
After the Stem Cell Therapy, 2-3 sessions of Hyperbaric Chambers are given to:
Mobilize and proliferate the production of newly administered Stem Cells.
Heal and recover the damages caused by degeneration.
Minimize the entry and survival of pathogens inside the body because a rich oxygen environment isn’t favorable for the growth of microorganisms.
Reduce the unfortunate events of infections and inflammation post-treatment.
Enhance the division or differentiation of the new stem cells into the required cells.
